As with any procedure requiring general and/or local anesthesia, there are certain risks that serious complications or even death may result. To minimize the risk of such occurrences, we mandate baseline bloodwork be performed in order to assure proper organ function, clotting ability, detect anemia or infection, and provide a baseline for future reference.

As the owner of the above pet, I certify that I am over the age of 18 and I authorize the staff of this hospital to perform the procedure(s) listed above, as well as those deemed necessary to treat life-threatening emergencies. I understand there are risks inherent in these services and that I am financially responsible for all costs incurred.

By signing this form, I acknowledge and agree to the following terms for boarding my pet at Central Animal Hospital:

1. Vaccination Requirements: I confirm my pet is current on all required vaccinations including Rabies, Distemper, Parvovirus, Bordetella, and Leptospirosis.
2. Unvaccinated Pets: I acknowledge unvaccinated pets are at higher risk of contracting contagious diseases and assume full responsibility for any associated risks.
3. Health and Safety: I confirm my pet is in good health at the time of boarding and authorize Central Animal Hospital to provide necessary medical treatment at my expense if needed.
4. Liability: I release Central Animal Hospital and its staff from liability related to the care of my pet while boarded, understanding there are inherent risks including injury, illness, or death.

I, the undersigned, am at least 18 years old, and I am the owner or duly authorized agent for the owner of the animal described herein.

I hereby consent to and order euthanasia (humane death) to be performed on this animal, forever releasing said doctor, hospital, and all agents from any and all liability for performing said euthanasia.

I consent to the veterinarians at Central Animal Hospital performing additional diagnostic tests and/or imaging as needed, including bloodwork, X-rays, ultrasound, heartworm testing, and other necessary diagnostics.

I understand these services are not included in the consultation fee and will incur additional costs. I accept responsibility for all approved diagnostics.

I authorize Central Animal Hospital to perform grooming services for my pet. I understand sedation may be required for the safety of my pet and staff if my pet is difficult to handle, and I consent to this if deemed necessary by the veterinarian.

I understand grooming may reveal or worsen pre-existing health conditions and release Central Animal Hospital from liability for such conditions. I agree to pay all grooming fees, sedation costs (if applicable), and any additional fees incurred.
