As with any procedure requiring general and/or local anesthesia, there are certain risks that serious complications or even death may result. To minimize the risk of such occurrences, we mandate baseline bloodwork be performed in order to assure proper organ function, clotting ability, detect anemia or infection, and provide a baseline for future reference.

As the owner of the above pet, I certify that I am over the age of 18 and I authorize the staff of this hospital to perform the procedure(s) listed above, as well as those deemed necessary to treat life-threatening emergencies. I understand there are risks inherent in these services. I acknowledge that staff members at this practice have explained the procedures to me, answered questions to my satisfaction, and cannot be held responsible for any unforeseeable results. I understand that I am financially responsible for all costs incurred during this surgery, treatment, and hospitalization.

While I accept that all procedures will be performed to the best of the abilities of the staff at this facility, I understand that veterinary medicine is not an exact science and that no guarantees have been made regarding the outcome of these procedures.

By signing this form, I, the undersigned, acknowledge and agree to the following terms and conditions for boarding my pet at Central Animal Hospital:

1. Vaccination Requirements: I confirm that my pet is current on all required vaccinations as outlined by Central Animal Hospital's boarding policies, including Rabies, Distemper, Parvovirus, Bordetella (Kennel Cough), and Leptospirosis.
2. Unvaccinated Pets: I acknowledge that unvaccinated dogs and puppies are at a significantly higher risk of contracting contagious diseases. Central Animal Hospital strongly recommends that all pets be fully vaccinated prior to boarding. By boarding my unvaccinated pet, I understand there is an increased risk of exposure to illness and agree to assume full responsibility for any associated risks.
3. Health and Safety: I confirm that my pet is in good health and free of contagious conditions at the time of boarding. In the event that my pet shows signs of illness or injury during their stay, I authorize Central Animal Hospital to provide necessary medical treatment at my expense.
4. Liability: I understand and accept that while Central Animal Hospital will take all reasonable precautions to care for my pet, there are inherent risks associated with boarding, including injury, illness, or death. I release Central Animal Hospital and its staff from any liability related to the care of my pet while boarded at the facility.

I, the undersigned, am at least 18 years old, and I am the owner or duly authorized agent for the owner of the animal described herein.

I hereby consent to and order euthanasia (humane death) to be performed on this animal, forever releasing said doctor, hospital, and all agents from any and all liability for performing said euthanasia.

It is my desire to provide for my pet decent and humane after-death care, complying with all legal requirements of the area. I authorize the attending veterinarian to dispose of the remains in accordance with hospital policy, releasing the hospital, doctor, and agents from any and all liability for performing after-death care.
